Abstract
Background: We investigated antipsychotic treatment patterns in patients with first-episode schizophrenia using the Health Insurance Review Agency (HIRA) database in South Korea.

Methods: The HIRA database between January 1, 2007, and December 31, 2016, was used for this retrospective population-based study. Treatment duration was defined as the sum of days from the first date of diagnosis to discontinuation. Discontinuation was defined as no visit within 28 days after the expected date of the next visit. Olanzapine equivalent dose of antipsychotics in the first episode and annual prescription percentage of antipsychotics were calculated. Proportions of patients with polypharmacy and treatment duration with polypharmacy were analyzed by year.
Results: We identified a total of 103,243 incident patients with first-episode schizophrenia which comprised 48,045 male and 55,198 female patients. The mean age of onset was 40.8 years. The olanzapine equivalent dose in the first episode was the highest in the young adult group and decreased in the adolescent and old age groups. The most prescribed antipsychotic was risperidone and the prescription proportion of atypical antipsychotics consistently increased over the
observation period. There was no remarkable change in the proportion of patients with polypharmacy but the proportion of treatment duration with polypharmacy increased during the observation period.

Conclusion: Antipsychotic dose in adolescents and old age was lower than in young adults and there was an increasing trend for the use of atypical antipsychotics. Treatment duration with antipsychotic polypharmacy increased as the illness progressed.
